Hi,
47 male, have been feeling strong pain in the perineum when sitting for 2 and 1/2 years.
The pain does not extend to the penis, scrotum or anus. Just the area between them and my glutes.
I also feel a strong pain right on this area. Can it be related to the pudendal nerve?
Can PN or PNE cause pain in this area?

Based on my own experience, I think it is possible that your pain is related to the pudendal nerve but you may need more diagnostics to know that for certain. My pain was primarily perineal also. I had upper/inner thigh pain as a result of the obturator internus muscle going into spasm. PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

Can anyone tell me if they have had excruciating leg pain with a flare? Burning pain all the way down to bottom of feet. So depressed. All I can do is lay in bed and cry from the pain.

What you are experiencing sounds extremely distressing. I have heard several people mention some foot pain on this forum, but what you are describing is not a typical complaint of people with pudendal neuralgia. So it would make sense to ask your doctor about it, as they may want to consider that there is more going on than just pudendal neuralgia. Is there anything that helps it, like heat or ice? What about medications such as pregabalin (lyrica) or duloxetine (cymbalta)?

A very knowledgeable PT I consulted with years ago advised me that when there are both PN-type issues and leg and foot symptoms, it is likely there is something else going on. One possibility you can ask your doctor about is an issue in the lumbar spine or sacrum. The pudendal nerve and sciatic nerve share the S2 nerve root so it's worth considering if there is an issue there cause you symptom of both conditions. I am sorry you are dealing with such severe pain right now.

PN started 2004 from fall. Surgery with Filler Nov. 2006, Dr. Campbell April 2007. Pain decreased by 85% in 2008 (rectal and sitting pain resolved completely), pain returned in 12/13. Pain reduced significantly beginning around 11/23.
