he did tell us he's only done it 4 times and that the most similar case to ours (age, onset, symptoms, etc) did end up helping. You should very seriously ask to talk to that patient and make sure that patient is at least 2 years post-op. What puzzled me on the facebook group is the number of relaps...

According to Dr. Jerome Weiss, the steroid which looks like milk in the syringe, crystallizes when it dries and that can by itself irritate the nerve. It is why he would switch to Heparin/Lidocaine injections for repeat treatments instead of a steroid.

Fentanyl patches are reservoir based. They can potentially be dangerous if damaged. Fentanyl is also much more addictive. Can't they prescribe Butrans (buprenorphine) patches? They worked great for me and around the clock. You replace the patch after 7 days. They also present virtually no risk for abuse (full agonist).
I can't believe they are not more popular in the US (originally from the UK).
