Hi Michaela, I had four injections for pudendal neuropathy last year, then had the decompression surgery in Sept of '10, with Antolak. I am 11 months post op and in worse pain now than before the surgery. I am also experiencing severe pain in my piriformis muscle and as a result, sciatic nerve pain. Those piriformis spasms may also be impeding the healing of the pudendal nerve.

I saw Antolak this week and he suggested I see a Physiatrist or Orthopedic specialist to look into a possible injection into my piriformis to calm it down. Not sure if the piriformis issue is related to SI Joint dysfunction, but it could be. I like Antolak, and on the one hand I tell myself to be patient and see if the nerve still heals (he told me 9-24 months). But living in this kind of pain daily, not being able to sit, can't work more than 4 hours a day (standing up at my desk), causes me also to seek out other potential remedies. I have been reading a lot about Dr. Dellon, but his methods have to be considered experimental at this stage, and I worry about possible long-term problems, even more than I have now.

Bi-lateral pudendal decompression surgery September 2010, Dr. Antolak. 54 years old, had radical prostatectomy in 2007 which may have contributed to this pain. Former runner, weight lifter etc. 10 months post op, very little improvement able to walk 15 minutes a day, little to no sitting

Hi Gregg and others,

Those are the potential problems that the patients are trying to the attention of the doctors.Dr Hibner,Bautrant,Antalok,Beco,Tibet and Conway have made some headway as far as Pelvic instability is concerned after these PNE surgeries especially after TG approach.


The main issue is the denial of Roger Robert of Nantes and the Houston who keep on denying that there is any Pelvic instability.The reason is best known to themselves.The ligaments are there for a purpose and it's an evolution of a monkey's or chimpanzees tail as being the main reason that we don't need those ligaments.I believe that the Piriformis muscle could be the result of transposing the nerve at ischial spine or the missing SS ligament.

PT for almost 3 years now without any results.
Pudendal Nerve blocks in August,2007 by Dr. Quesda left me with sitting pain.
Unilateral TIR approach with Dr. Bautrant on 18 Febuary,2010 with no major improvements and sitting is much worse.
MRI By Dr. Potter reveals nerve entrapment in the ST,AC and DN.
Dorsal Nerve Decompression surgery on April 8,2011
Redo surgery by Dr. Hibner on July 18,2011

If they weren't getting good results, they would stop. The internet forums have not convinced them they're not getting good results, apparently! 8-)

I'm certain that it doesn't bother them if patients choose to go elsewhere, so I can't fathom why it would bother somebody else. People have choices and will live with the results of those choices, so I'm just not seeing any issue.
PNE as a result of childbirth, 2002. Treatment by the Houston team, with neurosurgery by Dr. Ansell in 2004. My left side ST and SS ligaments were found to be grown together, encasing the pudendal nerve.

I am cured. I hope you will be, too.

There are no medical answers on the forum. Your only hope is to go to a doctor. I was very happy with the Houston team, which has treated the most PNE patients (well over 400), more than any other US provider.

I am still trying to make my mind up on which doctor I should see, Dr. Hibner or the team at Houston.I like the Houston team because a Neurosurgeon does the surgery(Dr Ansell). The only thing I don't like about the team at Houston is that Dr ansell cuts out the Sacrotuberous Ligament.What I like about Hibner is that he repairs this ligament.What I don't like about Hibner is that he a OB/GYN not a Neurosurgeon.
This is not going to be a easy decision to make.

Joddy,

You will regret having the ligaments severed by the Houston team.There are too many bad cases of Pelvic Instability and the PNE success rate is still not upto the standard.You might not improve at all and have a secondary issue like SIJD,Piriformis muscle syndrome,tailbone pain etc

Joddy, no one can predict what your surgical outcome will be. I wish you all the best with your decision and I hope you can find relief.

Thanks Celeste. I am still worried about severing the Sacrotuberous ligaments. I'm afraid that I will have pelvic instability if these ligaments are severed. I think you said you had these ligaments severed and did not have any problems. I am a male and that might make a difference when severing these ligaments.
