Violet,

I remember you saying you had the TIR approach. I have read that during the TG approach the pudendal nerve is transposed so that it is positioned in front of the ishial spine instead of behind. Do you know if the nerve is transposed in the TIR approach? Does anyone know how important this is????

I am on 100 mcg. of fentanyl with percocet for break though pain. I'm not on Lyrica because my blood pressure went through the roof when I tried it.

Paulette, the nerve is not transposed in the TIR approach. I can't say how important this would be in all cases but it does not seem to have been very important in my case since I have improved so much.
PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

You said the nerve is not transposed in the TIR approach. Was your SS ligament cut? If not, I do not see much difference between your surgery and the surgery Dr. Martin in Oklahoma performs. Thanks for the answers.

Paulette, yes the SS ligament on the right side was completely severed and on the left 2/3 severed.

That is where your surgery differs from the surgery performed by Dr. Martin. At this point he does not believe it is necessary to sever any ligaments, (according to his nurse). But it seems to be important to sever at least the SS ligament. I asked Dr. Conway if he transposes the pudendal nerve after severing the SS ligament. He said all of the major pudendal nerve surgeons transpose the nerve. In fact, the nerve in most cases transposes itself to the front of the ishial spine. I found that interesting.

What does transpose mean?
Athlete until pain started in 2001. Diagnosed with PN in Nov. 2010. Transpose means to move the nerve from where it is to some place else (usually into the fat tissue in the TG approach).
Mild to moderate PN for 5 plus years, pain controlled by lyrica and opiates.
Nerve block (unguided) 9/10 Dr. Jerome Weiss - sciatica for 5 months but got numb in painful perineal/scrotal area - he diagnosed entrapment - but no more cortisone for me
Potter MRI 5/11 - rt STL entrapment of PN at Alcocks
Consult with Dr. Hibner Feb. 2012
Bilateral inguinal hernias diagnosed by dynamic ultrasound - surgery on 6/20/13
Feeling a little better, a few more months will tell

Hi everyone

First post in a while, mainly due to holiday ahead of the surgery, which is happening with the Bristol team next Wednesday (12th October). Thanks for all the guidance provided so far, hopefully will try and put it into practice.

Will do my best to post again as soon as I feel able post surgery.

Woke up one day in February 2004 wth PN symptoms - cause not known. Numerous drugs, epidurals and nerve blocks (incl CT guided) later, PNE diagnosed in early 2010. Decompression surgery (TG) undertaken at Frenchay Hospital, Bristol in October 2011. Currently under care of Dr Gareth Greenslade, Pain Consultant, Frenchay Hospital.
