Nerve Ablation

Has anyone had nerve ablation done with alcohol injections into the PN after the PNER surgery? I still have pain upon sitting down manifested from the PN scar. Hibner is my surgeon and he hasn't done this procedure but his P.A. is going to speak to him about doing this procedure on me. He does it on other scars. He believes my issue is not neurological but muscular so I am not a candidate for a re-do. He is also going to consider trigger point injections into the scar area which he has not done in the past so if anyone has ever had luck with that, I would love to know. I am trying to avoid a scar recision situation which he wants to do.

What nerves would he do an ablation on exactly? Where exactly is your pain upon sitting? Has he mentioned anything about inferior cluneal nerve involvement?
-11/08 vulvodynia began around conception of first & only pregnancy
-3/10 sacral/sitting pain began after SIJD manipulation
-Progressive widespread pain- central sensitization
-PT, meds, injections, botox, ESWT = debilitated.
-5/12 Potter MRI - scarring of left ST, coccygeous & posterior alcock
-12/12 - left FAI/labral hip tear surgery
2014-2019 managed w/ gabapentin, massage, and lifestyle mod
2020 - big flare up

Are you considering an alcohol injection into the incisional scar or to the pudendal nerve itself - I can't tell for sure from your post. It's my understanding that it could kill the nerve and you could suffer incontinence if it's directly to the nerve.
PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

I have not met with Dr. Hibner himself to discuss either 1) nerve ablation or 2) trigger point injections around the PNE scar on my left glute. I had that initial discussion with Lisa Wadsworth last week. He has done nerve ablations into the ililingual nerves on 2 patients with success according to her but not to the PN yet. She is going to get back to me once she discusses with him these procedures. He did inject me (3 months ago) with marcaine and Kenalog into the nerve in one place to see if my pain would be lessened. That did not work so I am not hopeful about the trigger point injections. But, I want to avoid a scar excision if I can as involves such a long recovery. He did tell me I could swim after the surgery for exercise but that is it. I won't be able to sit on the left glute for at least a month; no squatting, bending, etc. at all. But, it may be worth it to get out of this high pain cycle. I can't push a vacuum, sit for more than 15 minutes, squat without very high pain which lasts all day long.
It wouldn't make sense to me to ablate the nerve itself, either as it serves 3 major organs. Once I receive the rest of the information, I'll post it.
