Ladies and Gentleman,
I have great news to share with everubody.Dr. Hibner operated on a patient recently and who had 3 Tesla MRI surgery with Dr. Potter and he found exactly what Dr. Potter had reported in her report.The PNE 3 Tesla MRI imaging by Dr. Potter has made the breakthrough that we were all looking for.

Hello all,
I am now ~ 70 hours post-op, R-side TG Decompression with Dr. Hibner...and want to relay some good news regarding the utility of MRI for the diagnosis of Pudendal nerve compression. The take-away from this post is that Dr. Hibner's surgical findings were "as-described and predicted" by Dr Potter's Pudendal MRI (see below post from 10/15/2010). Dr. Hibner was so elated that he called Dr. Potter immediately after my surgery, and they agreed to do research together (and to include Dr Hibner's radiologist at St Joseph's Hospital). Dr. Hibner referred to Dr Potter as a "genius" in her abilities to visualize and make assessments. I gathered that Dr. Hibner felt this is a diagnostic breakthrough in that he only wants to operate on patients that can benefit from his procedure.

I would add that one validation does not make a complete study or prove that my experience will translate to others. But wanted to let the forum know what just happened.

I will post in Case Update section in the coming days on my experience and progress. So far it has been great. Dr. Hibner and his staff are exceptional.

I am a Pudendal neuralgia sufferer (right side only), and have been formally diagnosed by the teams in Houston and Phoenix. After over 2.5 years of conservative treatment (the whole none yards), I am having the TG decompression surgery (Right side) in a couple of weeks.

Being a systems engineer, I have tried to leave no stone unturned in seeking as definitive a diagnosis as possible. Knowing from a previous MRI that I have suspect Right and Left labral tears and Femoral Acetabular Impingement (FAI) Syndrome, I wanted to study further - prior to Pudendal surgery - whether there was a link...i.e., between my perineal pain and the labral tears vs pudendal neuralgia. Adding to this suspicion, my wonderful pelvic floor PT indicated that there is growing interest among her peers regarding labral tears and pelvic floor issues/pain. (But even beyond implications of the labral tears, my hope was to leverage imaging capabilities mentioned by Ali P. and others recently in this forum to further the certaintly of the Pudendal diagnosis, regardless of its cause.)

So I went to NYC last week to the Hospital for Special Surgery for 2 MRI studies: bilateral Pudendal nerve, and right hip. Both MRIs were read by Dr Hollis Potter. And I was fortunate enough to talk with her briefly...to better understand her findings. Dr Potter’s insights were great, and not only confirmed my Pudendal clinical symptoms and findings to date, but also gave specifics into what is is likely going on in there.

From the Right hip study, Dr Potter did confirm labral tears and FAI, but indicated solid doubt that this is the cause for my Right-side perineal pain. The bilateral Pudendal MRI indicated a normal Left side (no pain there for me), but found the Right Pudendal nerve is compressed at 2 locations: sacrotuberous ligament (which is deformed and scarred), and by a prominent dilated vein in the posterior margin of Alcock's canal. And there were other lesser findings. The MRI Impression reads like a roadmap, and with GPS-like clarity. The impact for me is that I am now more confident that TG Pudendal surgery is the logical next step.

My plan is to work with my surgeon, reviewing the MRI and findings prior to surgery, and validating the MRI findings with his post-surgery observations.

Cyclist till 4/08 pain onset right side. Formal Dx w/PN via Houston Team 8/09. Examined by Dr Hibner and his PT (Loretta) 9/09. Lyrica and PT since 10/09 w/40% improvement. Dr Potter MRI 10/10. Surgery set for 11/10.

PT for almost 3 years now without any results.
Pudendal Nerve blocks in August,2007 by Dr. Quesda left me with sitting pain.
Unilateral TIR approach with Dr. Bautrant on 18 Febuary,2010 with no major improvements and sitting is much worse.
MRI By Dr. Potter reveals nerve entrapment in the ST,AC and DN.
Dorsal Nerve Decompression surgery on April 8,2011
