I went for a pudendal nerve block ordered by my neurologist. Drove 4 hrs. and found out they don't do them (told me on phone they did). Anyways, the x-ray my coccyx and did a ganglion impar block. I felt enough relief that I could sit but still felt level of burning in buttocks/groin so that wasn't my whole problem as we had hoped. I can say that I can stand to lie back on my back for a little while but my buttocks heat up and up. Coccyx area just not as touching as before (for now).
Symptoms began 6/27/12. Diagnosed with Vulvodynia.Estrace and Lidocane with no relief.
Uretha enlarged Aug. 2012. MRI of Lumbar in Sept. 2012.Normal. Did MRI 3 Tesla of ischial spine to Sacro Spin- Normal. Houston-Nerve Block 10-31-12 & MRI (MRN) entrapment of perineal branch, more on right side. Dr. Hibner- block of perineal branches and sclerozation w/relief over 1 month. SIJD/Piriformis Syndrome Left Side. Gabapentin 2400 mg.

I had PN relief from Ganglion of Impar (i.e., Walther) and L3 Sympathetic blocks, done on separate occasions, with local anaesthetics. Relief from each lasted several hours, and affected somewhat different, though overlapping, parts of the PN pain distribution.

Next, I had radiofrequency ablation in the same two areas (two locations within the Ganglion of Impar & bi-laterally at the L3 Sympathetic Ganglion), all in one session. Together with my intrathecal pump (morphine & bupivicaine), oral Lycica and Valium, I had no PN pain for three months. (I still had SIJD pain).

Since my doctor (Eduardo Quesada, M.D.) is critically injured, I have been looking for someone to do the radiofrequency. I have located two potential pain management specialists, and received the Bi-lateral L3 Sympathetic radiofrequency ablation/lesioning (it seems to temporarily damage the myelin sheath). It took a couple weeks to kick in (I have no idea why) last time. It's been a few days, and pain in my genitals seems to be lessening (hope it's not just wishful thinking).

Ganglion of Impar is an easier procedure to do, than the L3 Sympathetic Ganglion, so it's easier to find someone who does radiofrequency ablation to do it.

Hope this helps. Good luck.

Thanks for that info David.
Good luck, hope the wishful thinking becomes a permanent result :)

David, it must have been a real shock to have to start searching for another physician especially after you have been battling PN for so long. I hope these new docs will be able to help you.

Best,

Violet
PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.
