Hello Out there.
My pain is so severe this morning I may head on over to the ER and have them call my neurologist. This is an every morning "Feels like someone has kicked me in the scrotum". "Lumbar aching"
Any males out there experience this! You can see my first post in the Welcome section. Add to what you see there, prostrate is fine, epidimidis ruled out.

Dogwalker, did you have any success at the ER? I'm sorry things have gotten this difficult for you.

I think you said your pain is mostly on one side. I know you were doing PT for your hip but maybe you want to check with a pelvic pain PT or a manual therapist to have your musculoskeletal alignment and pelvic floor muscles checked out. If you suspect pudendal neuralgia don't let any PT's talk you into strengthening exercises though -- and be careful of stretches. You may also want to get an appointment with a PN specialist because the waiting list can be long. If you determine PN isn't your problem you could always cancel in plenty of time with the specialist. Just a suggestion.

Not sure what being kicked in the scrotum feels like since I am a female but I often felt like I had been kicked in the crotch - or as I used to say it felt like I had been hit with a baseball bat. Definitely not fun and I considered going to the ER before too.

Check out the pain medication section of the forum. It sounds like your pain is not well managed. Do you have a pain management doc?

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.
