I received my MRI report from Dr. Potter. The findings state small varices course with the dorsal nerves ( I can not see the veins) to the clitoris. There is increased signal intensity in the bulbospongiosus muscles bilaterally. A small portion of the sciatic nerves pierce the piriformis muscles bilaterally. Could these findings be the cause of PGAD and PN? If so what would you suggest my next steps be?

I don't know, I can't say for sure. There are people who have had varices (that seemed significant) embolized and it didn't make a difference. My previous gyn said she sees women with large pelvic varices and no symptoms at all. So I don't know if anyone can say with any certainty whether the varices are what is causing your PGAD. If I remember correctly, don't you have quite a few other symptoms besides just in areas innervated by the dorsal nerve? I wonder if that suggests a different etiology than just these small varices next to one branch of the nerve.
Before having anything done that is invasive and irreversible, it seems like it might make sense to at least have an evaluation by a good PT to get their input also.

Thank you! PGAD was before vestibuletcomy. I'm losing hope because if it's not the vein it leaves me with nothing to go off of. I won't know what steps to take next. I see my doctor on Monday and I am hoping he at least explores the vein as a possibility.

I've also read vulvar veins are rare if your not pregnant. Also they usually go go hand and hand with pelvic congestion syndrome. Which I don't have, per my MRI. I am thankful that nothing serious showed, but at the same time I was looking for the "it" that caused PGAD.

Well, I think it's way too soon to lose hope. You have a lot of different options so I think there is good reason for hope that you will get better.
