New Member-Seeking Advice

I am a 55 years old male from Northern Virginia and for the last 6 months I am having a lower pelvic pain. Since my pain started in the lower pelvic region, I saw several Urologists who treated me for all the prostate related conditions such as chronic prostatitis, BPH, Chronic Pelvic Pain Syndrome etc. So far I have taken a lot of medications and antibiotics but none of them has helped with my pelvic pain. I even had Prostate MRI done few months back and it came clean. I would like to mention here that I do not have any of the classic prostate related symptoms to begin with such as frequent need to urinate, increase frequency of urination at night, blood in urine pain with urination or ejaculation. I have even see a Gastroenterologist for hemorrhoids who also cleared me.

My pain/discomfort is in the lower rectum/anus area which I can describe as a soreness/numbness. The pain always stay in the same region which is lower rectum and near the opening of anus so it is localized. I get up every morning 100% normal with no pain. The pain will only get intense once I start sitting and putting pressure on my pelvic like driving to work, sitting on chair or hard surface etc. The pain is more intense in the morning and in early afternoon hours and less intense in the evening. I can avoid getting pain by remain standing or laying down which I pretty much have been doing.

I am so relieved to find this forum and hoping for some advice and guidance. After reading few of the postings, I am certain that I am suffering from Pudandal Neuralgia or some form of it as all my symptoms fit the description of this condition. I would be delighted to hear from other members and looking forward to their advice. I also would like to find Doctors in my area (VA, DC, MD) who specialized in this area so any assistance will greatly be appreciated.

If you want to investigate PN further with doctors, I suggest Dr. Rachel Rubin a urologist in DC or Dr. David Gordon a neurourologist in Maryland. Dr. Richard Marvel is a gyn and pelvic pain doctor in Baltimore who also sees men. If conservative measures are working to relieve your pain, such as standing instead of sitting, I suggest continuing with that. I hope you are able to figure out why you have pain.

PN started 2004 from fall. Surgery with Filler Nov. 2006, Dr. Campbell April 2007. Pain decreased by 85% in 2008 (rectal and sitting pain resolved completely), pain returned in 12/13. Pain reduced significantly beginning around 11/23.

You could also check out the list of physical therapists on our website at pudendalhope.org to see if there is one near you. If you have muscle tension in your pelvic floor you might get some relief from myofascial release. Could be worth a try at least, before doing anything more invasive.

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.
