To keep it simple, I dont know if my case is the only case in the world with this issue! But here it goes. A few months ago, I was put on a catheter for 5 days. During those 5 days, I was on bed most of the time, walked only a few times, but majority of the time I was in bed. This is an important thing to note, which I will explain now below.
Once I got off the catheter, I noticed immediately that I had this "block" or "obstruction" underneath my penis (it is a small area, just underneath near the base) where whenever I pee, it pinches. Essentially, imagine if you are trying to force water out through a pipe but you try to choke one side, preventing the water to come out less or sharper. Thats exactly what is happening - if I peed, there is that one area in my penis that would try to choke from underneath the penis, and it will cause me to pee but have this feeling of obstruction or blockage.
This went on for a few months. Things started to get a bit better after I started going to a pelvic floor specialist. However, that same problem still remains. Went and did a cystoscopy, urologist said everything is ok. Scheduled a few sessions with a dry needling/acupuncture specialist, who dry needled my lower abdomen, my lower back, thighs, glues, hamstrings and also the abdominal muscles to get a release. So far, the issue looks to be a blood flow problem - the lower abdomen whenever I sit down causes me to have aches and weird burning sessions. We dry needled that area near the bladder, and it was very itchy. This could be a blood flow issue.
So far, that pinching underneath my penis is still there. If I can explain more, I notice that if I am sitting for longer periods, or, when i wake up in the morning, I see that pinching underneath my penis when I urinate. This has caused me to wake up a few times at night too where I pee very little...it feels like the problem is getting worse now.
I really hope I was able to explain this to all. My question is this - what exactly is the muscle or nerve that controls that penile muscle underneath the penis? Is there a way I can tell the specialist where to do a "release" or dry needle which will help stop the pinching once an for all? We are trying to figure that out or trace our way back to where we can do a release so that the pinching goes away. This looks to be something going in my lower back or my hips...but again, I have no clue what muscle it is that is causing this. My guess is that when I was catheterized, one of these pelvic floor/lower abdomen/hip muscles got tightened up, and stayed tight.
I don't know if I am making sense, but I am at wits end at this point. I would greatly appreciate if there is someone who can advise what the specialist can focus to help deal with this issue. Or, if there is a specialist I should go to that can help me with this issue. I don't know which doctor would be that would help here - the urologist that I saw did not have any answers.
I can even share some pictures of my penis with that pinching when I pee, but I think that may be graphic! But I can share if that helps. The example I gave with the pipe being choked in one area whenever the water flows would be the best way to describe it. And it is always pinching in that one area, never in other areas.

Bulbocavernosus (also called bulbospongiosus) muscle, or ischiocavernosus muscle? I know of one person who had problems peeing after having a catheter in. Over time they developed a stricture that eventually led to acute urinary retention which is a medical emergency.

You would think if structyre is your problem, it would have been found with the cystoscopy, so it seems unlikely a stricture is your problem, but I'm just telling you this in case you do end up having acute urinary retention at some point -- you would need to seek immediate medical attention.

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.
