Just my 2 cents. Since Dr. Castellanos is a 1st year fellow in the program Dr. Hibner teaches in, I honestly don't think he would be performing the surgery solo. Hibner would HAVE TO BE THERE, by law, I do believe.
Daughter grew completely on left side of pelvis
Multiple uterine surgeries to fix uterine adhesions, septum, and endo
Had all the conservative workups done, 3Tesla (Potter), recovering from L sided TG (Hibner) 11/10, Botox 6/11 failed, bilateral anterior PNE decompression (distal Alcock's/perineal branch), Aszmann, Vienna, 10/11; dx'd with CRPS Type 2, 12/11, Ketamine @ CCF 2/12, doing 75% better PRAISE JESUS!

Faith, I glad to hear that Dr Hibner's PT (Loretta)takes Medicare. I am disable&on a fixed income.

Joddy,

Don't take my word on that. It may have been Medicare not Medicaid. I didn't listen too closely since it didnt' apply to me. You can call them and ask them though.
-11/08 vulvodynia began around conception of first & only pregnancy
-3/10 sacral/sitting pain began after SIJD manipulation
-Progressive widespread pain- central sensitization
-PT, meds, injections, botox, ESWT = debilitated.
-5/12 Potter MRI - scarring of left ST, coccygeous & posterior alcock
-12/12 - left FAI/labral hip tear surgery
2014-2019 managed w/ gabapentin, massage, and lifestyle mod
2020 - big flare up

I managed to squeeze in an appointment with Loretta before I see Dr. C in April. I downloaded Loretta's paperwork and it says they bill for Medicare. There are some other notes about primary and secondary Medicare insurance also. You can find the information under forms at www.desertpt.com . Hope this helps!!

Have been dealing with burning pain since Jan 2010.
No sitting since April 2010.
Seen the following dr's: DO, GYN, Dermatologist, Accupuncturist,
URO GYN (his RN is the one who suggested the pain could be PN), Neurologist
Had ECG and MRI both inconclusive, only the SSEP said Pudendal reaction was abnormal and they lost that test result.
Saw Dr. Castellanos April 6, 2011. Next steps, MRI and botox. Having PT while waiting.-Botox denied, appealing to Insurance company now. :(

Hello All,
I have seen dr. castellanos twice now for clitoral nerve blocks and genofemoral, hibner did my botox
anyway none gave me much relief. I have sharp glass like sensation in my clitoral area and cannot sit at all. too much walking makes this bad too. no meds are working for me/.
Dr. casteallnos called me as I have been here from ireland for nearly 9 months now/
I am on medicare.
Can anyone tell me who has been on medicare here, I have no secondary insurance, but I have to get a ct guded pudendal block into the buttock now in the hope the whole nerve can be got and this will also get to the clitoral part of the nerve.
Has anyone on medicare got this ct guided block done in phoenix at st josephs and if so can you tell me what admissions charged you
I am on medicare and have written checks to them now totalling nearly 1,500. they ask me up front all the time here and in mri office also

So i am wondering what will they charge me here in admissions........I have never never paid any hospital in the usa upfront while I was on medicare why does this hospital ask me for money up front every time I go in here. Now they owe me a credit for 1,400 approx cos my botox was covered and they did not refund me just starting taking the other fees I owed out of it, I am on disability and nearly broke here and what i get does not cover me to have my own place to live it barely covers anything. can anyone tell me will I have to pay anything cos no one in st. joes hospital will tell you until you arrive there on the morning of your admissions and appointment =======thankyou lexie
