Susi, I'm glad you are feeling a bit better. There are many ways to combat constipation so I hope you can get that under control. What have you tried so far?
PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

Thought I would post an update.

I stopped increasing my nortriptyline at 75 mg due to constipation, which has been relieved now with daily fiber. I definitely think the nortriptyline has helped.

My pain: overall, I am in less pain than I was when this started back in March. More specifically, I have less constant, unprovoked pain. On the other hand, I have more sitting pain (a deep ache just inside my left sit bone). I basically cannot sit without a cushion; this started around May. The pain is along the entire course of the nerve and its territory - different places at different times. Sometimes it is an ache over the left sacrum, sometimes it is the sit bone pain, sometimes it is labial burning, sometimes it is left inner thigh burning, sometimes it is a prickly pain in the whole perineal area, once or twice I had rectal burning. I'm usually pain free in the morning, and on good days, I'm pain free until night. Lately I've been having runs of 7-10 good days, followed by a few bad days. Sitting in the car and driving for an hour usually causes pain.

Pain is not as much a problem for me as how it affects my life: I cannot sit without a cushion, and I cannot have sex.

Would it be crazy to consider surgery at this point?

I'm starting to consider it. See updated signature. I'm still hoping this will resolve on its own. But I'm also starting to move ahead with seeing specialists.

Still haven't had any nerve blocks or testing. I know what my diagnosis is, and I don't have much faith in any of those things helping me. Am considering a PT evaluation, but again, I don't want to irritate my nerve and cause more pain than benefit.
Vulvodynia diagnosed '01; symptom was occassional vulvar itching/rawness after sex.
Autumn '10, localized clitoral and left labial burning after sex
March '11, new left buttock pain, worsened clitoral/left labial pain/burning.
May '11, increased sitting pain.
Summer '11, pain decreased/plateaued to 0-4/5.
Treatment: 75 mg nortriptyline nightly, definitely helps; PT x 4, didn't really help.
Consults with Hibner and Dellon, left me with questions.
Next step: MRI in NYC or VT.
