I haven't had Botox injections so I can't speak from experience but I know that typically they would inject Botox into the muscles that are determined to be in spasm, so where they inject Botox might depend on what muscles a pelvic floor PT finds to be in spasm if they do a pelvic floor exam. Often with pudendal neuralgia the obturator internus and some of the levator ani muscles are in spasm because the pudendal nerve runs between those muscles.

Historically on this forum, there are mixed reviews on Botox. Some people find it helpful, while some people have flare-ups from it unfortunately. I'm sorry you did not get good results from your injections.
