PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

The reason that i wanted to read Northern Spy's posts is that my symptoms don't match the classic hernia symptoms in that i dont get relief lying down. But i have had this for three and half years so maybe some of the pain is central sensitization. I do get MORE pain from any pressure into my groin: sitting, squatting, massaging the area, walking etc. And, i have had a lot of PT and therapy where they were pressing in that area to break up scar tissue from my labral repair surgery -- which was constantly aggravating the area and generating pain. I have an appointment with a hernia surgeon this friday and i have an appointment with dr meyers later in the month.

Don: have you considered doing cryoablation? Maybe your ilioinguinal nerve is just really irritated from years of pain? I am also going to look into this.

Hi Bev,
Just a suggestion regarding the ilioinguinal pain:
If you haven't had recent imaging of your spine, you may want to do that to check for T12 - L1 facet joint pathology. However, it's rare that something significant will show up in the study. It's more common for radiologists to report "insignificant" findings. A qualified PT or chiropractor may be more helpful with a hands on exam:

Ultra Sound in 03/08 showed severely retroverted, detaching uterus with mulitple fibroids and ovarian cysts.
Pressure and pain in lower abdomen and groin area was unspeakable and devastating.
Total lap hysterectomy in 06/08, but damage was already done.
EMG testing in NH in 04/10 - bilateral PN and Ilioinguals
3T MRI at HSS, NY in 09/10
Bilateral TG surgery with Dr. Conway on 03/29/11. Bilat ilioinguinal & iliohypogastric neurectomy 03/12. TCD surgery 04/14.

Karyn, that post is really fascinating to me because my pelvic PT actually found I was having pain in my facet joints somewhere and did some of those stretches shown in the link. It's a real thing!
Started with vulva itching 2/11. Diagnosed as vulvodynia and then PN in 01/12. Progressed to sitting pain and constant burning. Received 4 nerve blocks with only temporary relief, did year of pelvic PT before being diagnosed with bilateral FAI and hip labral tears 7/12. Did orthopedic PT 1.5 years. 3/13 Arthroscopic hip surgery at HSS with Dr. Kelly. Take cymbalta and lyrica daily. About 97% better than worst point thanks to combo of surgery and meds. Yay!

At this point, things seem to be improving so I will wait and see what happens.

Nerve block (unguided) 9/10 Dr. Jerome Weiss - sciatica for 5 months but got numb in painful perineal/scrotal area - he diagnosed entrapment - but no more cortisone for me
Potter MRI 5/11 - rt STL entrapment of PN at Alcocks
Consult with Dr. Hibner Feb. 2012
Bilateral inguinal hernias diagnosed by dynamic ultrasound - surgery on 6/20/13
Feeling a little better, a few more months will tell

Hi Karyn: thanks for the post -- interesting, my symptoms don't match the description but i will bring it to my PT (who is the same as jackie's). I don't have an MRI above the lumbar. But L1-L2 has some degeneration. The ilioinguinal and genitofemeral come out of L1-L2. Anyway, i know i have narrowed it down to these nerves as when i press on them i elicit the pain -- all the way down to my vuvla and clitoris - it is pretty straightforward. It also explains why all the pelvic floor PT i have been doing hasnt helped because they were focused on the pudendal nerve only. Weirdly, when i mentioned this to my PT, that she is barking up the wrong tree -- she keeps saying i just need to strengthen my hip and keep doing pelvic PT?? It is a one size fits all for alot of these peeps. On a side note, i hope i didnt have unnecessary hip surgery -- and instead it was a hernia!!

I have my hernia consult on friday. He is supposed to be the top hernia guy in NYC. Thanks Don for all the info. BTW, Don, Did they give you PT post hernia operation? what is the protocal from hernia rehab -- pre and post.

I wish you the best with your hernia appointment. Hoping you get some answers! I don't know for sure if your hip surgery was unnecessary or not, but it doesn't sound like it.
It's possible to have hip pathology, PN and other neuropathies as well.

Do you see the same PT for your hips and pelvic floor? I don't know how she can dismiss the rest of your body.

My PT is a hip rehab pelvic floor specialist -- so she does both.

I went to my hernia consult. He couldn't feel a hernia but said that you need to do a CT scan with vasalvar (sp?) etc to be 100% sure. He wants me to rest and take motrin as he thinks i injured the ilioinguinal ligament. I am doubtful of this because i already have rest and taken anti-inflamatories but i am losing my fight, am worn down and depressed. Anyway, i am going insist on the CT scan when i have the energy.
