Hello all. You have all given me so much help so far. I am still unsure if I have PNE, but many of my symptoms seem to fit.

At the moment I feel 'broken'. I was wondering if anybody else felt like this.....

It seems I permanently need to have a BM. I also permanently need to urinate.... is this normal?

I basically feel that I could do either at any point in time if I tried.... my sensations about when I actually need to 'go' - whether to urinate or have a BM - seem to have completely deserted me....

Is this common... my BMs in particular seem so heavy, all the time.

I feel broken, too. You are not alone. We all need to help each other with this because very few understand.

Hi J,

Yes, I felt broken for a long time too -- felt like I needed to urinate most of the time. No, this is not normal but it's common for people with pudendal neuropathy because the pudendal nerve helps control bowel and bladder functions. PNE surgery has pretty much cured the problem for me.

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

I guess part of my problem is that I don’t definitely know whether I have PNE or not. Much seems to fit, but not everything.

Most people tend to state urinary problems. I have these, but they are rivalled by bowel problems. And there are things I just don’t understand.

For instance, if I have PNE, why does diet affect me? Whatever I eat, I have trouble passing stool. Normally I go 3 to 4 times a day, spending half and hour each time, but the process can be exacerbated by the food I have eaten the previous day. Alcohol also has a bad impact.

Other things I don’t get are:

- Why do I get so much pain in stomach/abdomen after a BM? Normally BM’s make things easier don’t they?
- Why do I feel sick and dizzy after a BM?
- Why are my stools so weird? They are generally very mushy, which again suggest diet is a problem. Sometimes I can push and nothing comes out. An hour later I can pass large sticky stools!
- Why does having a BM often lead me to needing another one? I can feel comfortable beforehand, first thing in the morning, but then after a BM all hell can break loose!
- Although I take a long time to have a BM, food does pass through me very quickly. This seems a massive contradiction!

I am also worried about the end game! Where will this lead and what can I expect. Sometimes it feels like something has to give…. Can you break your pelvic floor somehow? Can it pop!!?

If you go just by the symptoms you just described above, then yes, it does sound like maybe you are on the wrong track thinking it is PN but if you add in your original post, PN is not such a far-fetched idea. Maybe you have more than one thing going on as so many of us do.

My main concern is my love for running. I have been a long distance jogger for several years... this isn't the cause of my problem as I have been ill from my youth, but if it complicates things I may have to give it up. I was working out in the gym at around the time things got worse as well......

The weird thing is, the only time I really feel OK is after exercise, no doubt cause of the endorphins. Perhaps the negative effects are not felt straight away?

Any thoughts would be brilliant.

Hi ive had things like prostitis/ irritable bladder/urge inco plus bowl things like you mentioned to a degree?. Was in urology gave me things/tests etc usual crap?. don't do this/drink that blah blah. need to do pelvic excerises etc.Now think my back/spine is problem so gave him this page/site to chew on that stumped them!.
As like you have a real pain in the butt and always feels like i wanna crap but never sure what to do and gets painful to do as seams even when it wants to fall out you still have to strain.
One thing that can be cause/aggrivate it is gym work with weights/stretching out/rowing etc as one reason it effects alot of woman is because of child birth displacing hips etc.
I still cycle but have found in certain positions i'e when sitting sometimes forward/putting pressure in a certian area i get nerve shocks that taser you in the goolies bersssst! but also when you sort of crouch down like a wide leg sitting sort of position when your like trying to see something low down.
so must put pressure onit or stretch it?.

hopefully you have gotten your BM issues under control
I have learned that the problems you have described are classic PN issues

unfortunately the PN is more than sensory, it has motor responsibility the the external anal sphincter muscle, which is critical for normal BM function

you very likely are have spasms of the external sphincter muscle due to the PN damage

you need to do whatever it takes to avoid constipation..constipation will just make the PN damage worse
do whatever it takes to get unconstipated..miralax for example is very effective..dosage needs to be adjusted so that stool is not to hard, but not to soft either..add fiber to diet, try everything

longer term, you need to eliminate the spasms, at least when you need to go

try nitroglycerin ointment..it relaxes the muscle to allow more normal passage of stool...colorectal surgeons prescribe it all the time for anal fissures..can be applied before you go, and other times during the day

biofeedback has also been shown to be effective in retraining you to learn to relax the muscle that is in spasm
