Think Pain Specialist will do the injection?

I have a pain specialist that i kind of built some rapport with that knows my situation well.
We thought it was due to the back and I tried some epidural shots w/ fluoroscopy.

Do you think he would have any problems trying the nerve blocks described on this site?

nobody here would know the abilities of your doctor

ask him if he has done them before, and how many he has done

you want the injection from the most experienced you can find

I got my nerve blocks from a local pain specialist. He had not in fact done a pudendal nerve block before. He had done many other kinds of nerve blocks and had a reputation among local Doctors as being very capable with a needle. When I spoke to him about it, he was iffy at first but then researched the topic and decided he would be interested in doing it. I was a satisfied customer: although the nerve blocks did not have long term therapeutic value for me, just as they do not for most people, they did have diagnostic value and, after a minor flare, some short term improvement. I have seen patients on this site complain of much more arduous nerve block experiences, with much more experienced Physicians.
pelvic pain started 1985 age 14 interstitial cystitis. Refused medical care from age 17, did GREAT with self care for years.
2004 PN started gradually, disabled by 2009. Underlying cause SIJD & Tarlov cysts
improved with PT & meds: neurontin, valium, nortriptyline, propanolol. (off nortriptyline & propanolol now, yay!)
Tarlov cyst surgery with Dr. Frank Feigenbaum March 20, 2012.
Results have been excellent so far; but I won't know my final functional level for a couple of years.

My PM outright admitted he had no experience doing them. Plus, you want them to be guided, either ultrasound or CT. Do you live near a big city where you could find someone experienced?
I did have to travel 4 hrs to Denver to get mine done.

great that the injection worked for you, but it really comes down to this regarding choosing a doctor for injections

you want the doctor with the most experience doing exactly the injection you are going to get
no different than having a more major procedure, surgery, etc.
doing these injections is not rocket science, but it is a skill that like any skill, will improve with repetition
the doctors with experience have already bumped into every possible complication and will have learned how to best avoid them
so regardless of the personality of the doc, or the patient/doc relationship, please everyone, go with the one with the most experience, there is no substitute

just my opinion of course, but I think most here will agree

I agree in theory, but in practice most of us have limited funds and no PN specialist in our backyard. I just wanted to share that I have managed to get a lot of good care locally and that is a reasonable option sometimes. For example Dr. Filler wanted to do piriformis injections on me out-of-network to insurance, which financially would have been like financing a car; Almost any Pain Clinic Doctor does those routinely as an in-network procedure. When it comes to surgery, doing the "PN world tour" really becomes necessary; for more minor procedures I think there is often an advantage to checking into local resources first.

I had the MRN in NY, but through Filler cause I thought it would give the best result

I had already had numerous scans which did not show the problem

Filler's MRN's cost me a bundle being out of network, but I don't think I wasted my money since the scan had findings that none of the other scans showed...problems that I knew I had with my PN's and some dilated veins which regular MRI's did not reveal either

so with those findings I am in the process of finding the right treatment..so far some meds, but probably headed for something more invasive either to treat the veins or the PN problems
The pain specialist that is giving me the meds also does the injections, and he is local and in-network, so if I wind up with that, cost should be reasonable

the doc who would treat the veins in also in network

horrible symptoms for all of us, along with finding the rights docs, treatments, etc.

people like yourself and the rest of the participants on this forum have been so valuable, I cant thank you all enough!
