INCISION PAIN AFTER 3 MONTHS

hi
as you all probably know i have had pn surgery, it is now 3 months since the surgery and the pain is still as bad even more so some days, however my incision site looks really good but it still very tender and aches a lot, i wondered is this normal? i would ask the surgeon who did the op but as of yet he has not returned any of my emails or calls although his secretary has asked him to ring me i am still waiting

I have a lot problems with my incision since my last pudendal surgery. Felt like shards of glass in it, cutting with razors when shower water would hit it and couldn't stand to have anyone touch my butt. (Poor husband :D ).

The pain clinic I go to ordered a cream with Ketamine/Lidocaine/Gabapentin that is made at a special local compounding pharmacy. It really has done the trick. As long as I put it on 3-4 times a day it rarely bothers me. Might be something to ask about.

I've heard cold laser therapy helps incisional pain. Haven't tried it myself so I can't say from experience but it might be worth a try if nothing else works.

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

Are you doing any pelvic PT? I remember my incision being really sore for months; couldn't put any pressure on that butt cheek for a long time. Dr. Hibner had me resume PT at 6 weeks postop. She started doing very gentle scar massage which got more aggressive as tolerated to help keep scar tissue from forming into a hard ridge at the incision, which causes more problems. Your PT no doubt has ultrasound and other tricks to try. You might see what a pain mgmt. physician can offer, but get clearance from your surgeon before doing any injections. One of my former pain anesthesiologists performed scar injections of some kind (not on me).
Acute left vag/rectal pain 1999 lowering into chair. Dx 2006 with PNE/VV by PTs, Dr. Garcia, Weiss, Hibner. Many shots. PNMLT: left PN barely fuctional. Left TIG by Dr. Hibner 2007 (ST lig). 75% improved 1 year postop, slow decline. Pelvic floor Botox 2010. Left sacral SCS 2011 after good trial; removed 2012 due to NEW left S2 & rectal pain, bowel freq. Re-do left TIG by Dr. H & 5-day ketamine 2013; 1-1/2 years postop 70% improved. New post-SCS Sx are worse than PN Sx.
