Update, New 3T MRI

Hello Everyone,

Just an update.

Been up and down these last few months. Some better days some worse. I have yet to find any patterns. Here is my most recenet 3T scan. I am supposed to go for injections, but i am really worried about making things worse.
Not sure why i have scar tissue and edema, i never had an accident or fall. Could this be what's causing my issues?

My PT said i have good control over my pelvic floor muscles, and they don't seem too tight. But i have not really seen any progress so i am hoping she is not making things worse in there.

HISTORY:
Pelvic Pain-MRI Pelvis without contrast-Pudendal neuralgia-Miriam Bredella to oversee MRI -


REPORT MR neurography of the pelvis

TECHNIQUE: Magnetic resonance imaging of the PELVIS, focused on the
pudendal nerves, was performed without intravenous contrast using MR
neurography protocol.

COMPARISON: MRI femur 2/3/14, MRI pelvis 12/17/13, MRI lumbar spine
12/17/13

FINDINGS:

No mass or fluid collection is noted along the course of bilateral
pudendal nerves. There is minimal scarring/fibrosis adjacent to the
right pudendal nerve in Alcock's canal. No signal abnormality of the
pudendal nerve is identified. No muscle atrophy is seen within the
pelvis and bilateral piriformis muscles are symmetric. The hamstrings
insertions are normal.

Mild marrow edema involves the right medial inferior pubic ramus.
Bone marrow signal is otherwise normal.

IMPRESSION:

MR neurography focused on the pudendal nerves demonstrates minimal
scarring in the region of the right pudendal nerve.
Bone marrow edema of the right inferior pubic ramus may be
post-traumatic.

Where did you get this done? In Boston or did you go to NY? The bone marrow edema in my experience really means nothing. Not sure about the scarring at the PN. Where are you getting your injections? I have one scheduled at B&W but I'm canceling. They just don't listen to what you have to say and then it takes endless months to get an appointment.

PN started 2004 from fall. Surgery with Filler Nov. 2006, Dr. Campbell April 2007. Pain decreased by 85% in 2008 (rectal and sitting pain resolved completely), pain returned in 12/13. Pain reduced significantly beginning around 11/23.

I had a ganglion impar because I was told I did not have PN. Did nothing. Then as I was being rushed out the door DrV said we will do a pudendal block next. This was the end of March and the 1st injection is June 10th. I have an appt with a different pain clinic. I went to see Dr Stewart and she believes I have PN and wants me to see a physiatrist. She'll be the 3rd one in the last year...not really sure why I am bothering.

Do your symptoms match an "entrapment" at Alcocks?where was your MRN performed?

Hibner consult 10/11 w/ plan: 2 mo. PT
No meds work for me
PRF X 3 times in Denver ( was pain free for 5 months after second)
