This post is to the moderators, nurses or a patient who use baclofen or valium, .
I have severe rectal pain due to a spasm in the anal sphincters-- severe PFD( Pelvic floor dysfunction)
I have an option to add baclofen to my pain pump.. along with the current meds -dilaudid and bupivacaine ( The latter not helping much )
The baclofen is to help with muscle spasm which my phys therapist feels is the true cause of my pain. I agree as I hold my stress and fear in my pelvic floor--- a catch 22 ie,knowing if I relax, I could get better, makes it harder for a worrier like me to relax those muscles!
So my pain mgmt doctor has rx'd the baclofen.. He does not know me well and tends to jump on the first question I ask at a visit,
come up with an idea and send me on my way..He never spends more than 10 15 minutes in a visit..So I tend to feel uneasy about his decisions as they have been given little thought.. We never get to all my questions as he is off and running etc...
Back to my question...
I also use a combo of Balcofen 5%- valium 10% combo anal rectal suppositories..They help with pain while lying down but don't stay in while upright and induce the urge to defecate. They do also help with urinary urge at bedtime. I like the relief I get while lying down.
But the problem: the doctor is going to take away my suppositories if he puts Baclofen in the pain pump.
That is an option I don't want to risk as I don't know. whether the Valium or the Baclofen ... provides the pain relief?
or the combination?
So I changed my mind after thinking about it over the holidays and also was advised against it by my PT who feels, along with my urologist, that a Local med to the pain area as opposed to systemic Baclofen, is better for me. She knows me better than my pain doc.
She and I have spent an hour together weekly for almost 2 years and she is the only provider who has helped me.,She understands pelvic pain clearly ...and has done much study o fit.. So I tend to go with her logic. I am still in pain using my combo supps since they don't work while walking around, but my PT and I have much work to do in the painful spot starting now..
we avoided the area for a couple months as I was busy and didn't want the flare it causes, but we must work on the area to get back to the progress we were making in the past. (long story)
Does anyone use Valium suppositories separately? ... or Baclfoen supps? or do u take either of these meds orally for muscle spasm?
Just trying to see which drug is the helpful one in order to make the best decision.. To be clear, I don't get intermittent spasms.. My anal muscles are "stuck" in a spasm.

Kathy, I used oral baclofen for awhile after PNE surgery. It didn't really help with symptoms much -- just kind of made me feel weak as I recall. I tend to agree with your PT but I don't really have enough experience to say for sure one way or the other. I guess you won't really know unless you try it. It might be worth a try if you are sure you have the option of going back on the suppositories if the baclofen pump doesn't work.

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.
