Does an Alcock Canal nerve block reach the distal nerves?

Just wondering if a block to Alcock's canal reaches the distal nerves (ie., dorsal to clitoris, rectum, etc) better than the one to the ischial spine.

Not sure you will get a definitive answer, but will relay what I've been told by the PN docs. One cannot distinguish where entrapment might be by a block, be it at the ischial spine or into Alcock's. The tissue planes are continuous between the two areas. Not a complete answer at all for you, but related info.

Cyclist till perineal pain onset R side (Apr'08). Dx with PN (Aug'09). Lyrica gave 30% pain reduction. Potter MRI (Oct'10) validated at surgery with Dr Hibner (Nov'10) - found nerve attached to scarred STL. Now sitting 10 hrs/day vs 1 hr/day pre-op. RJR, I have been told differently by PN dr.'s. Dr. Westesson does blocks at both locations and I can tell you from having both areas injected that the different areas made different parts of me numb.
2/07 LAVH and TOT 7/07 TOT right side removed 9/07 IL, IH and GN neuropathy 11/07 PN - Dr. Howard
6/08 Obturator neuralgia - Dr. Conway 11/08 Disability, piriformis syndrome - Dr. Howard
4/09 Bilateral obturator decompression surgery, BLL RSD - Dr. Howard
9/10 Removed left side TOT, botox, re-evaluate obturator nerve - Dr. Hibner
2/11 LFCN and saphenous neuralgia - Dr. Dellon 2/11 MRI with Dr. Potter - confirmed entrapment
5/11 Right side TG - Dr. Hibner 2012 Left side TG - Dr. Hibner

Theoretcially, a nerve block should numb everything distal to where the nerve block is given but in reality it doesn't always work that way because nerve blocks don't always hit the target with 100% accuracy. The rectal nerve does not branch off in the same place in everyone -- there are variations in anatomy so whether or not the rectal branch is numbed by an alcock's canal nerve block might vary. Like RJR said, the sites are very close together and the mediation wanders so it's impossible to say for sure with any accuracy where the entrapment is.
PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.
